Using the Adobe Acrobat Reader
There are several forms on this site that can be downloaded. These files are prepared using
the Adobe Portable Document Format (PDF) program.
You can read and print copies of these forms by first downloading the Adobe Acrobat Reader.
This Adobe Reader may be already installed on your computer as a part of the operating system,
or as part of your web access programs. If it is not installed, you can download and install
the program for your operating system and hardware at no cost from the Adobe Site. Just click
on the Adobe Icon to the right, or enter the following URL address in your browser window.

For most systems, an installation wizard will be created that will help you through the
installation process. All PDF files have a name this is of the form name.pdf, where
.pdf is the extension name. If you have installed the Reader program correctly, calling a
file with this extension will cause the program to start with the file included.
