material. Please read the Knox Co. Board Policy if you are unsure
as to whether or not you should complete the forms below.
BHS Band Medical & Release Form
Knox Co. Medical Form (This form is NOT optional
for band students, it is REQUIRED due to the travel we do.)
Knox Co. Asthma - Self Possession of an Asthma Inhaler
by a Student Form
Knox Co. Physician Form for Administration of
Medication and Self Medication Administration Form
Important Note: This includes EpiPens and other allergy related
medications.
In order to best meet your child's needs please make sure that the
appropriate paperwork is completed and returned to the band office ASAP.
