PACKAGED TOGETHER, BUT EACH ENTRY MUST HAVE ITS OWN ENTRY FORM AND A FEE IS DUE
FOR EACH ENTRY IN EACH CATEGORY.
Tearsheets preferred for print entries;
clippings or photocopies of published material are acceptable if tearsheets are not available, but clips should be pasted to
an 8.5 x 11 sheet of paper. Submit CDs or audio cassettes for radio entries and
DVDs or VHS video cassettes for television entries. Label entries clearly and securely attach entry form. For categories that include supporting material posted online, include a complete URL for online entries.
