The purpose of the East Tennessee Lions Eye Bank Gift Program is to give individuals, families, and organizations the opportunity to make tax-deductible monetary donations to the East Tennessee Lions Eye Bank. Money donated will be used to help fight blindness and eye diseases not only in our region but throughout the world.
To make a contribution, you may print out this form below and complete all applicable information. The Lions Eye Bank, at your request, will mail you a receipt for tax purposes.
