Team managers will need the following information:
- Copy of Birth Certificate (if you played with Fusion FC last season, then you team manager has this document)
- Medical Release (must be notarized and the notary commission must be valid until 6/15/10)
- Photo Release
- Small Photograph (for player pass)
- Player Agreement
- Bristol Release - This is a waiver from the city of Bristol, TN that must be signed by all players.
