Team managers will need the following information:

• Copy of Birth Certificate (team manager from last year has)
• Medical Release (must be notarized and the notary commission must be valid until 6/15/09)
• Photo Release
• Small Photograph (for player pass)
• Player Agreement
• Bristol Release - This is a waiver from the city of Bristol, TN that must be signed by all players.

NOTE:  Interstate Release for VA Residents only.  Please complete the form and mail it to Vicky Pinto, 408 Coralwood Drive, Kingsport, TN  37663
